Oldham older people's community mental health team

Our service is offered to older people with severe and enduring mental health issues, both organic and functional illnesses.

Referrals come via the Single Point of Entry and all clients must be open to the Older People’s Consultant Psychiatrist.

Assessments are offered to identify need and support plans are designed to ensure that individual need is met.

This can be done by arranging services that will support an individual to remain as independent as possible. Financial assessments are arranged as there can be charges for services.

We are based at the Royal Oldham Hospital and it is rare that visits are made by the client to the office. Visits are usually based at home or mutually arranged destinations.
