Our service transformation programme is well underway across our boroughs of Bury, Heywood, Middleton and Rochdale, Oldham, Stockport, Tameside and Glossop.

We want to keep people out of hospital by improving and expanding our community services for people of all ages and better supporting families and carers. To achieve this, one key focus is strengthening partnership working as part of the new primary care networks and building on this to develop wider living well services in each borough.
We’re focusing on better supporting people experiencing a crisis, so they can avoid hospital. We’re strengthening existing services and exploring new approaches, working closely with colleagues, patients, carers and a range of partners.
We also want to ensure people who need hospital care can be safely discharged or stepped down as quickly as possible. This will rely on a lots of different professionals working together, involving the patient and improving our planning and processes to eliminate unnecessary waits and delayed discharges.

Much of our transformation work is possible through additional national funding. However, in common with NHS organisations nationally, we’re still required to make financial efficiencies. This is a real challenge, but our staff are working hard and we're making steady progress.
