Stockport Palliative Care in Dementia Liaison Service

We are part of the wider mental health liaison service for older people.

We provide specialist mental health assessments in a variety of health and social care settings, in order to ensure your mental health needs are supported throughout the last 12 months of life.

We aim to ensure colleagues in palliative care services are able to access specialist advice and assessment for patients with dementia, or with a life limiting condition with cognitive problems.

We also provide support and assessment for carers and relatives.
