Tameside home intervention team - older people

We work with older adults who require intensive home support and treatment, in order to avoid admission to a hospital bed and maintain you within a community setting.

We also facilitate timely and safe discharge and follow-up from the older people's mental health inpatient wards (Summers and Hague).
